Abstract

Purpose: To assess the interobserver reliability of standard radiographic parameters in non-operatively managed distal radius fractures.

Methods: Two independent hand and microsurgery specialists evaluated the radiographs. Reliability was assessed using intraclass correlation coefficients (ICCs), Cohen’s kappa, and Bland–Altman plots.

Results: Continuous measurements, including ulnar variance, dorso-volar angulation, and radial inclination, demonstrated excellent reliability (ICC > 0.90). The categorical evaluation of associated ulnar fracture yielded substantial reliability (κ = 0.79).

Conclusions: Continuous radiographic parameters can be applied mostly with confidence in both clinical decision-making and research settings, whereas categorical assessments may warrant more cautious interpretation.
